Lisa Marie Berquist Polk (June 16, 1970 – April 19, 2014) of Cumming, GA went to be with her Lord and Savior Jesus Christ after many years of illness.
She was a loving, beautiful daughter, wife, sister, mother, grandmother and aunt. Her whole life was always dedicated to what she could do for others. She will always be loved and missed by all.
Lisa is survived by her husband, Mike Polk of Cumming, GA; parents, Carl & Joanne Sharp of Cumming, GA and Art & Gail Berquist of Mountain Home, AR; brother, Todd (Vanessa) Berquist of Mountain Home, AR; sisters, Carlene (Ken) Jones of Cumming, GA and Bethany Jenkins of Dahlonega, GA; children, Allison Dickerson, Jacob Sharp, Charlie Polk, Molly, Polk, Ethan Polk and Emily Polk; grandchildren, the pride and joy of her life, Bentley (21 months) and Ava (16 months); niece, Jordan Murdock; nephew, Justin Rothrock; cousins, Michael (Judy) Palmer of Crown Point, IN and Julie Berquist of Chicago, IL.
She was preceded in death by her uncle, Lawrence (Henrietta) Palmer of Crown Point, IN.
