SALEM – Shirley L. Bazidlo, age 89, died at 9:43 a.m. Monday, April 23, 2018 at the Salem Regional Medical Center. She was born April 11, 1929 in Du Bois, PA, the daughter of the late Charles and Eileen (Dilly) Siple.
Shirley was last employed at Salem Regional Medical Center for 27 years in the dietary department. She was a member of the St. Paul Catholic Church.
Her husband, Frank E. Bazidlo, whom she married November 19, 1959, preceded her in death June 9, 1991.
Survivors include a son, John Bazidlo of Salem; a daughter Tina (Larry) Welton of Salem and a grandson.
A sister, Yvonne S. Levandoski also preceded her in death.
