Patricia Polomchak age 63 of Fair Oaks, IN, passed away on Monday, February 22, 2010, at Autumn Hills Health & Rehab in Roselawn, IN. She was born on October 1, 1946, to the happy union of Edward and Nava Jo (Daniels) Sizemore.
Soon after her birth, the family moved to Gary, IN in the Glen Park section. They lived in the same house for over 30 years later moving to DeMotte where Patricia eventually followed. She was raised Catholic and attended St. Joseph the Worker parochial school before graduating from Lew Wallace High School, Class of 1965. In January of 2000, she married Charles Kenneth Polomchak, who survives. Patricia will be mourned by many who knew her.
She is survived by: her husband, Charles Polomchak, Fair Oaks, IN; daughter, Paige (Robert) Rivera, Islamorada, FLA; mother, Nava Jo Sizemore, DeMotte, IN; grandchildren, Joshua, Mika, Robert Jr., Madelyn all of Islamorada, FLA; brother Edward Sizemore, Jr. of Kirkland, WA & sister Sheila (Robert) Loosemore, Portage, IN; step-children, Stacey & Michael Polomchak, Fair Oaks, IN; nephews, Charles Jr., John, Jason and great-nephews, Brice & Ethan niece, Janel and great-nieces, Nici, Jocelyn & Serena.
She was preceded in death by her father, Edward.
