Chastity Ann Puhak, 26, of Polk City, Florida passed away on Tuesday, September 10, 2013, of heart failure.
Chastity was a very lively spirit, she loved to make you laugh and smile. Her laugh will linger in the soul of her friends and family for all of eternity. She had a beautiful soul and voice, now she will sing with angels in Heaven with her late Nana, Beverly Ann Puhak.
Chastity is survived by her parents, John David Puhak and Nancy Harris; grandparents, Joan Harris and Victor Puhak; sisters, Shawna Nageli and Lisa Puhak and numerous nieces, nephews and cousins.
